Forms

Publish a set of questions, collect responses, and review them as summaries, tables, or CSV.

Last updated: 2026-10-15

What it does

Forms is the feature for lining up questions, publishing them, and collecting and viewing the responses. You can use it for inquiries, event attendance, surveys, quizzes, and more. You can check responses inside Addness as a summary, a list, or a CSV, and if you link a form to a goal, you can also open it from the goal's page.

To open it, go to "More" (the left sidebar on desktop, the bar at the bottom of the screen on mobile) → "Forms" in "Other features." If you use it often, pin it in "Other features," and on desktop you can open it right away from the left sidebar. In newly created organizations, it may show "Open with a challenge" (→ Understanding the sidebar and screens).

Create a form

Your connected AI can do these steps for you.

  1. Press "Create a form" at the top right of the Forms screen to create a blank form. You can also start from "Contact," "Event RSVP," or "Feedback" in "Start with a template"
  2. In the "Question" tab, enter the form's title, description, and questions. Add questions with "Add question", and set the type, required, and description for each question
  3. Use "Preview" at the top right to check the screen respondents will see (answers entered in the preview are not submitted)
  4. Press "Publish" at the top right to start accepting responses

Your edits are saved automatically (you can also save right away with "Save" at the top right). In "Defaults for new forms", you can turn on "Collect email address" and "Make new questions required" from the start for forms you create from now on. You can bring in questions from other forms with "Import questions from other forms".

In the form list, you can search by name and duplicate a form with "Duplicate" on each row (this copies the questions, settings, and theme into a new draft; you can also choose "Share with the same collaborators").

Question types

TypeWhat it is
Short answer / ParagraphA short one-line answer / a longer multi-line answer
Radio Button / Checkbox / Pull-downChoose one / several / one from the options. You can also add an "Other" free-text option
Linear scale / RatingRate on a numeric scale, or with star, heart, or thumbs icons
Multiple-choice grid / Checkbox GridChoose in a table of rows and columns
Date / Time / Date and timeEnter a date or time. You can also choose a date without the year, or a duration
Upload FileThe respondent attaches a file (respondents need to sign in)
Title and description / Image / Youtube Video (none of these require an answer)Insert a description, image, or video with no answer field

With "Validate Answer", you can set conditions such as character count, number range, number of choices, or a regular expression. You can also write the text shown when the condition is not met (the validation error text).

Sections and branching

With "Add section", you can split a form into multiple pages. For Radio Button and Pull-down questions, you can choose which section to go to next for each option (only one question per form can branch).

Publish and share the link

When you press "Publish", "Share" appears at the top right. From "Share," you can copy the following.

  • Public link: The URL of the response screen. You can also make it a short URL with "Use a short link"
  • Embed HTML: Paste it into a web page to show the form
  • Pre-filled links: A link that opens with answers already filled in (files cannot be pre-filled)

If you fix questions or settings while the form is published, the saved changes appear on the response screen as they are.

Decide who can respond

In the "Settings" tab, under "Who can respond", choose the "Response access".

  • Anyone with the link: Anyone who opens the link can respond
  • Organization members: Only members of the organization can respond. Respondents need to sign in to Addness
  • Selected members and groups: Only the members you choose and the participants of Addness groups you belong to can respond. You can also set an access expiration for each member and group

When the form is published to "Selected members and groups," you can use "Notify people with access" in "Share" to send an Addness notification to the people who can respond (messages up to 1,000 characters). When an organization member who was not selected opens the form, "Request access" appears, and the form's owner chooses "Approve" or "Decline". A declined request can be sent again after 24 hours.

View responses

Open the form and press the "Answer" tab. There are four views.

  • Summary: The number of responses, how responses changed over time, and a summary for each question (charts can be copied)
  • By question: See the answers to one question lined up
  • Response table: See responses one row each in a table
  • Individual: See responses one at a time

You can narrow the period with "Filter responses by date and time". You can print responses with "Print all responses". Attached files can be opened from within the response.

To find out about new responses, turn on "Notify New Answers" in the "Settings" tab. The form's owner gets an Addness notification, and also an email if email notifications are on. Collaborators can receive them too with "Notify me of new responses".

Export responses as CSV

Open "Export response CSV" from "CSV" in the "Answer" tab.

  • Output format: Choose "Question and choice names" or "Question IDs and raw answers"
  • Period: Enter a start and end date and time to export only the responses in that period (leave them blank for all)
  • Destination: Choose "Download" or "Save to Drive". A CSV saved to Drive holds the responses at that point (later responses are not reflected). You can save up to 50 MiB to Drive

With "Connect to Analytics", you can sync responses to an Analytics dataset. After you connect, it updates about every 5 minutes (→ Analytics).

Stop publishing

Use the buttons at the top right of the form to change its publishing status.

ActionResponse screenTo undo
"Close responses"The link opens, but a closed message appears and people cannot respond"Reopen responses"
"Unpublish"Opening the link does not show the form"Republish" (with the same link)

If you set a "Response deadline" (date and time) or a "Maximum number of responses" in the "Settings" tab, responses stop there. You can change the text shown when responses are closed in "Closed-form message".

What you can do in Settings

These are the main things you can set in the "Settings" tab.

  • Collect Responses: "Collect email address" (the respondent enters it / records the verified email of the signed-in user), "Limit to one response per person" (responding requires sign-in), "View progress," "Shuffle questions," "Disable saving draft responses" (normally, input from signed-in respondents is saved for 30 days)
  • View aggregated response results after submission: Show respondents the response count and the summary of choice questions. If you turn on "Share written answers too", people who can access the form can read the free-text answers even before submitting
  • After answering: "Allow editing responses after submission" (respondents can fix their response from an edit link shown once after submitting), "Show link to submit another response," "Email respondents a copy," "Confirmation message"
  • Linked Goals: Choose the goal to link the form to (→ Link to a goal)
  • Appearance: Change the accent color, background color, font, text size, and header image

Make it a quiz

When you turn on "Mark as Quiz" in the "Settings" tab, you can set the correct answer, points, and feedback for each question.

  • "Release scores": Choose "Manually publish after grading" or "Publish when auto-scoring is complete"
  • "Results to show to respondents": Choose "Only total points" or "Results and feedback per question," and decide whether to show missed questions, points per question, and correct answers

For responses you grade by hand, enter points from "Individual" or elsewhere in the "Answer" tab, press "Save grades", and show them to the respondent with "Publish results".

Link to a goal

You can link a form to a goal. To link it, you need edit permission for that goal.

  • From the form: Search for and choose a goal in "Linked Goals" in the "Settings" tab
  • From the goal: Press "Create" in the "Form" section of the goal's page to create a form linked to that goal. With "View all", you can see a list of only the forms linked to that goal

The goal's section lists the linked forms and the status of each (published, closed, unpublished, draft).

What you can ask the AI

If you ask an AI connected via MCP (Claude Code, Codex, and others), it can do the following for forms you can edit (→ MCP integration guide).

  • Create and duplicate forms, fix questions and settings, and import questions from other forms
  • Publish, close responses, unpublish, and link to a goal
  • Read the response list, a single response, and the summary, and export responses as CSV
  • Grade quizzes and publish the results
  • Set collaborators and transfer the owner
  • Delete forms and responses, and restore them from the trash

Who can edit (permissions)

These people can edit a form and view its responses.

  • The form's owner (the person who created it)
  • Collaborators: The owner chooses members in "Collaborative editing" in the "Settings" tab. You can set an access expiration for each member
  • People who can edit the goal the form is linked to

Only the owner can change the collaborators and the owner. If you make another member the owner with "Transfer ownership", you become a collaborator.

Delete and restore

  • Deleting a form: "Delete Form" in the "Settings" tab moves the form and its responses to the trash. Within 30 days, you can "Restore" it from "Trash" in the form list. If you restore a form that was published, its public link works again. "Delete permanently" in the trash cannot be undone
  • Deleting responses: You can delete responses one at a time, or with "Delete all responses". Deleting responses cannot be undone

Limits and restrictions

  • A form can have up to 300 questions and up to 75 sections
  • A file upload question accepts up to 10 files per response, up to 50 MB each. Respondents need to sign in to attach files
  • Images must be JPEG or PNG, 5 MiB or smaller, and up to 4096px. Only YouTube videos can be embedded (video files cannot be uploaded)
  • A form can have up to 100 collaborators
  • Forms work the same on every pricing plan
